In order to
participate in the Consolidated Office Systems
2012 Cash Rebate and Loyalty Rewards Program, a
customer must elect and meet the level of
participation.
Please read all the requirements and criteria
below and complete the
Application Form.

or hand deliver it to your “Business Product
Specialist”. All of the following requirements
and criteria must be met at the end of business
day on any quarter period ending in order
to receive your “Cash Rebate”. This offer may be
renewed automatically at the discretion of COS®.
(Any sign-ups received after 2012 - purchases
will be pro-rated from the date signed)
Criteria:
- Submit to
Consolidated Office Systems a credit
application, unless you already have an
active and approved open charge account.
- Maintain
your open account within our established
credit terms of “Net 30 Days” or 10th of the
month following receipt of statement.
- Qualified
purchases are those that count towards
regular, promotional, printing and services.
(City, State, Federal, Gov’t Purchases and
special contracts are excluded
- Purchase a
minimum of $1,000.00 per month averaged out
over each quarter and based on (12) month
period of this promotion.
- The monthly
average will include any credits or charge
backs against your account.
- Satisfy all
other terms and conditions as specified in
Consolidated Office Systems credit
application.
- Cash Rebates
earned will be paid out on a quarterly basis
and payable by the 25th of the month
following the end of the quarter.
- Checks will
be made out directly to the Organization’s
name provided on this form only.
- Cash Rebates
are based on purchases excluding any
applicable sales tax.
- The person
signing this form must be an authorized
agent of the Organization named on this
form.
- Bronze
Member status requires monthly purchases of
$1,000.00 to $2,500.00 per month.
- Silver
Member status requires monthly purchases of
$2,501.00 to $7,500.00 per month.
- Gold Member
status requires monthly purchases of
$7,501.00 to $15,000.00 per month.

purchases of $15,001.00 and over.
